Form responses

Operations about Form Responses

A form response is a patient's completed form, submitted through the API. POST /form_responses records the answers in NexHealth and writes them into the patient's chart in the health record system as form data. No PDF is made.

These endpoints are on v3.1.0 (beta). Sandbox keys can call it; production keys need access for your organization. See Versioning.

  • Form schemas exist for Eaglesoft and Denticon only; those are the two systems whose forms NexHealth reads. On every other system, including your Open Dental sandbox, GET /form_schemas is empty and there is nothing to submit against.
  • Get the form's id from Form schemas and send the answers in patient_submitted_data, keyed by the form's field names. Keys the form does not have return 400.
  • The request body can be up to 2 MB. Compress images before sending them as base64.
  • The write is queued when the call returns. The patient does not need a foreign_id yet; NexHealth creates the patient in the system as part of the write.
